Auto-populating form fields

I'm working on a form right now and am having some issues that I'm hoping someone can help me with.

I'm trying to develop a form that auto-populates specific fields.  Something similar to the "request an update" feature, but in a form that I can edit the look and layout of.  I've tried using a query string, but so far I haven't found a way to use existing cell data vs inputting the information to be displayed.  Additionally, the query string won't allow me to automate the form distribution because I have to edit each link before it goes out.

If anyone has any suggestions, I'd appreciate some ideas.

    Depending on your specific use case it maybe could work to create individual links automatically.

    Would that be an option?
