Employee Test Tracker

Smartsheet community, I'm looking to create a form for employees to fill out weekly to report their covid-19 test results. This needs to be weekly, and I need to track who has submitted and who has not for each week, probably via a report. What is the best way to create this form and report to keep track of employee testing. Thank you!
