Contact list within Forms

Hi,

 

Could someone help with the attached image.

 

I am hoping to have a drop down list of only the contacts names (removing the email addresses) as it currently looks too messy.

 

The contacts are required for alert and notification purposes.

    If it helps we have a Smartsheet Holiday request Form and approvals sheet or even a choice of... which might be helpful for you to save time if you would like a share?
