How to remove "Fill out forms with the Smartsheet app" from top of form?

I created a form, and emailed the form link to several persons.  I want the persons to simply click on the form link, which will open the form,  then complete and submit the form.

To keep the process simple, the persons do not have the Smartsheet app.

When the person clicks on the link, at the top of the form the user will see the statement "Fill out forms with the Smartsheet app." and a link to DOWNLOAD the app.  Rather than simply filling out the form and clicking the submit button, some users are downloading the app.  This is causing confusion, and users are requesting app IDs, etc.

How do I NOT display the "Fill out forms with the Smartsheet app" and DOWNLOAD button, at the top of the form?
