Identifying Batches Of Survey Results

Scenario:

When a team leader is signing off of their contract, a survey is sent out to each of the team members to evaluate their leader based on simple [5 – very good] through [1 – very bad] questions. Each question has the rating 1 through 5.

When the team leader signs off, the managers review the results of the survey with them to see where they may need to improve.

The Problem:

These surveys are submitted via a Smartsheet form and sit in a single sheet. For each team leader, there will be multiple rows which has their name and scores for each of the questions. The number of rows is the number of team members who submitted the survey.

However, when the team leader goes to another team months later and completes another contract, the same survey is sent out again (this time to a different team). When these results come back, other that the date which they were submitted, there is nothing distinguishing them as the 1st, 2nd, etc survey conducted on this team leader.

What We Are Looking For:

On a very basic level, we are looking for an automated method of identifying which survey a certain batch of results are. Keep in mind some of these surveys overlap with others so it may not be that rows (example) 50 – 60 are all the same team leader. The ideal graphic would be a line graph for each question – rating on the X axis, count of responses on the Y axis – and each line is identified as the 1st, 2nd, etc. survey values.
