Incorrect header - how to change alerts

Hi all: Having an issue with my SmartSheet. I have created dividers for dates, and have created an alert to say that when an item is approved, people are notified. Well, the notification has been including another line (with a previous date on it) and I have no idea how to fix it. Any advice? Screenshot attached. Thanks!
