Match COLLECT

Hello,

I need a formula that compares two columns and returns a value in a third column if there is a match. For example, if there is a Match of Jake, Smith then it will return the dept name

Name Dept Lookup Name Dept

Jake, Smith Science Jake, Smith (Return Value)

So basically something like this?
