Collect Date

Hello everyone, i have been trying to find a way to collect dates from a column that follows some criteria. Tried with index collect but i obtain always the 1st date that follows the criteria and when trying to incorporate a 2nd criteria i got incorrect argument.

I have prepared one test to show what I want:

What I try to do is fill out the Date Phase 1...6 with a column formula.
