Invoke a template from a new record

Use case: I would like for a user to complete one form, and based on a value they select in the form, it logs that entry into a sheet, and also creates another sheet (a template) with a name taken from a cell in the form.

I don't know if this is possible. But it sure sounds like a good idea! Some users on here have been able to use a paid account with Zapier. Maybe you could integrate both resources to get the job done? https://zapier.com/
