Check for Duplicates

We have a data sheet that we use for tracking requests, and sometimes duplicate requests are entered. I'm hoping to use a formula to flag rows that MAY be duplicates so that they can be reviewed and cleared if needed. For the sake of not over-triggering the check, I'd like to reference multiple columns, namely a "Name" column and a "Date" column (these are the column names). That is, if the value in the "Name" column AND the date in the "Date" column match, I'd like to have a checkbox column "flag" as positive to trigger a check. Can anyone help me with that?
