Certification Tracking

I am looking for a way to track all of our employees certification across the company. I would like to use a form with multiple selections but the form is turning out to be too large. I don't want the employees to have to make an entry for each certification either.

Things we are trying to track CERTIFICATION NAME, CERT #, ISSUE DATE, EXPIRATION DATE, ACTIVE/EXPIRED.

Thoughts...ideas?

What is the maximum number of certifications someone would need to enter at once?

@Paul Newcome There are hundreds of certifications we track. I would say on average an employee would have 10-15 certs.
