Work Flow "Unable to Run"

Hi all. I've built a number of sheets with multiple work flows. On this sheet, for the first time, i've experienced this error message. When i open the sheet, i don't see any indication of what is causing the error. I've rebuilt the work flow and the associated DocuSign Template multiple times and i get the same result. Any idea how to diagnose this or if there are common causes for this error? See attached screenshots of SmartSheet and DocuSign.
