Index collect with checked cell

The formula works fine, but what I really want to is that the IP (in column3) would only appear when the check mark (at column2) is checked. Is there anyway of doing that?

Can anyone help?

You would start it off with an IF statement.

=IF([Column2]@row = 1, INDEX?COLLECT(..........))
