Why two person gets same email for one automation

Hello!
We are using forms and automation. But I encounter problem. Here it is. Why is that?

It is difficult to understand what is happening with the information provided. But it's probably a problem with the automation configuration and not the form. There is certainly some kind of trigger being fired in both situations. You have to map the triggers to identify where the overlap is.
