Create sheet automatically fro template when rows are added

Hi,

Need assistance in creating sheets automatically when rows are added on the intake form. Also Department field in intake form has dropdown option of multi-select.


Based on the option it has corresponding columns in Template sheet that holds value of yes or blank. When dept 1 and dept 2 is selected . Rows that has dept 1 Yes OR dept 2 Yes needs to be editable and rest of them needs to be locked.
