Product Formulary used as an Order Form

Greetings. Fairly new user here.

I have a customer formulary of product SKU's and descriptions.

I want to create a form - linking all the SKU's available. If they select a SKU from the formulary, then I want it to "find" and populate the description and then ask for a qty.

I tried watching a view videos with using Reference Numbers but I couldn't get the formula to work.

You won't be able to do the lookup in the form itself. You can get them to enter the SKU and qty and then do the lookup after the form is submitted to the sheet. Other third party form apps may have this feature available.
