Document Generation is adding fillable forms in PDF

I am almost to the end of this task from a couple smartsheet forum question, (lol). I am trying to build a generated PDF as normal with linking of the fields with what fields should go where. For some reason however now Smartsheet is adding fillable form fields that are not on the PDF for some reason?

The sheet is empty with no attachments, adobe is not showing these form fields anywhere. It is almost like its adding these form fields from past document form fields I created in the past. I have reuploaded and renamed this PDF to see if they would go away but they keep re appearing and I am at a lost. This is the only PDF within the sheet mapping also.

This indeed doesn't seem to be expected. I’d recommend double-checking the fillable form in Adobe to ensure everything is correct before importing it into Smartsheet for mapping. Additionally, try the following steps to troubleshoot:

• Use a completely new sheet to test if the issue persists.
• Try using some sample data to generate the document to see if those fields still appear after the mapping.
