Workflow - Acknowledgement with Submit option

Hi Team

I have a scenario wherein a Form has been created to be submitted by Manager to provide feedback to their team member. All we need is that once the manager submits the form, it should get routed to the team member who get a notification, can access the form and "Check Box" acknowledgment and hit submit.

The form has been completed and contact field for team members is created for routing.

However need help with Notification Alert for Acknowledgment and Submit so that we can record if the team member saw the feedback. A simple alert to mailbox may not work since there is no tracking.

Can you please help.

Thanks Paul for the suggestion. I have configured the same however;

1.Is it possible that I can make all the fields that manager has updated as "view only" and only the Acknowledgment "Checkbox" as editable

2.The other downside is that Acknowledgement checkbox cannot be made mandatory for Submit but I can put a text next to it so that it sounds like one..

Please help provide insights on #1 and #2
