Form for evaluation

Hi,
I am the manager of our internal Survey Programming team. In Smartsheet I have one sheet where I put all information about each project we are running. Each row is a project. Now I would like to conduct a survey after each project is closed with our researchers (there is an assigned to column for the researchers) to see how satisfied they are with our performance. How can I do it so that the information is entered in the same row as the project?
Can I send out a form where researchers fill out the evaluation?

An update request would be the most straightforward way to accomplish this. To use a form would mean a second sheet and formulas on the first sheet with cross sheet references.

I have been thinking about an update request as well. However, I would like to ask in the update 'How satisfied are you with the overall performance of our Survey programming team on this project'. The issue here is that the number of characters is too big to have as column title. Is there another way to do it? In a form, the column title is not the same as the question itself.

Using a form, you would need to set it up on a separate sheet. You can then use a formula to create a hyperlink to the form with pre-populated fields (there are a number of threads on URL Queries here in the Community to help you get started with that). One of those fields should be some sort of unique ID for the row. You would set up an alert on the main sheet to send them the link generated by the formula.
Also in this main sheet would be the answer columns and each would have formulas with cross sheet references such as INDEX/MATCH to pull the form responses back over into your main sheet.
