Trying to Create Lookup Form

I'm trying to do something that is complex and new for me, and I'm also not very comfortable with Smartsheet formulas yet, so I appreciate any help with this.
Our company has a lot of Learning Plans for our staff based on Service Line, Level of Care, sometimes Job Title, whether the person is a new hire, etc. Our Onboarding Team wants a quick way to tell who would be assigned which Learning Plans.
I have Dashboards in Smartsheet for all of our different roles. What I would like to do is have a form that our Onboarding Team or any staff person could complete where they fill in the form with all applicable selections (for example, Florida Assisted Living Direct Care Staff), and the correct Learning Plan Dashboard link would appear.
So far, I have:
1. Created a Dashboard Learning Plan for all staff.
2. Created a master Smartsheet lookup table that lists every possible combination a person could fill in with a link to the appropriate Dashboard (all fields are text, not dropdown at this time.)
3. Created another Smartsheet with the same values as dropdowns.
4. Using the dropdown Smartsheet, I created a Form that will populate the appropriate fields based upon selection. For example, if someone selects Senior Living for the Service Line, they will then see a dropdown for Level of Care.
5. I created a helper column on both of my Smartsheets where it combines all of my columns into one summary (Senior Living|Assisted Living|Florida|Direct Care|No).
Where I'm stuck is figuring out how to make the two Smartsheets speak to each other where, once someone completes the form, the Learning Plan box on the form will display the Learning Plan Dashboard URL. I've been trying to use the Index and Match functions, but I just keep getting "no match."
Is there a better way to do this?!?

Unfortunately I have a sad news. Without Bridge implementation you will not be able to create from values in Sheet the dropdown list for column in another sheet. Its not out of the box solution that Smartsheet provides so you would need to play with Smartsheet Bridge to get it.

Thank you, @kowal I had a feeling that was the answer. I think what I actually need is the modern equivalent of Microsoft Access. I'm going to poke around our Microsoft 365 tools and see if I can use that for the lookup tool and link them to my Smartsheet Dashboards.
