Surveys Created from Forms for Public Access

Hi, I created a survey from Forms and the intent was to email it to my org's former donors for feedback. I looked at the form on my phone, I saw the message at the top of my phone about downloading the app or signing up. Since this is a survey/form created to be sent to people outside our organization, is this something that can be deactivated for this type of usage only?
I've filled out Smartsheet forms in the past from other businesses, and I never got a message about downloading the app or starting an account. So there has to be a way to do this... any ideas? I'm afraid it's going to confuse some people and make them feel as if they cannot complete the survey without downloading the app or getting a Smartsheet account first. ☹️

I figured it out, you have to disable the footer! 🙌

@Netra Check the security settings on the form. Is it set to require a Smartsheet login to access form?

Hi @Isis Taylor all of those security options are toggled off. I actually just figured out how to disable that message to download the app, I think you and I were typing at the same time, LOL!
You have to turn off the Smartsheet footer. I toggled with the footer and saw that this made the difference. 😀
