2 Criteria to Index Match

Hi

i wrote this formula to index/match with 2 criteria. I want to pull the name from this source sheet.

And have the name here in this sheet.

=INDEX((COLLECT({Local Crew Name}, {Position in Local Crew Sheet}, Position:Position, {PPI Local Crew}, Department:Department, "PPI", 1)))

I am sure there is an easy fix to this, but I am missing it. Thanks Smartsheet community!
