Automation sent to wrong person

I have an automation that has been working for almost a year. It sends a notification to the person who submitted the transaction via a form (from Contact List column). Yesterday the notification went to a person not associated with the transaction- their email was nowhere in the information. Any ideas on how this might have happened.
Our instance uses a Microsoft Azure identity management connection, but we do not have a full integration with our Active Directory.
The person it sent to has no affiliation with Smartsheet (license or shared user).

Hi @Kim Stephens, I have a couple questions to help us debug this.
- Has this only happened one time, or has this happened multiple times?
- Does this only happen with this particular email address?
- Can you try to replicate this problem and see if it happens again. In other words, fill out the form again, exactly the way that cause the error, and then let's see if the email gets sent to the same wrong person again.
I'm guessing that the problem is on the Microsoft side of things. For example, Microsoft might be confusing an email alias or something. But we'll need to dive into the 3 questions above to have a better sense of it.
