I currently have data which is in sheet 1 of my sample sheet attached. I am expecting an output in the format I have made in Sheet 2. This appears to be a Google spreadsheet, as opposed to a sheet in the Smartsheet application. This is the Smartsheet community, and our focus here is on working with Smartsheet. If you have a Smartsheet related question, you're welcome to edit your post to reflect that.
