I don't know of any similar template, but it looks straightforward to create one from scratch. I can help you if you'd want to. It does look pretty straightforward. At first glance it seems like the biggest "challenge" would be incorporating formatting since you can't merge cells in SS, but even that seems like it would be a pretty simple workaround with this particular form.

Maggie we have been looking after the NHS in Scotland for years now doing this type of work, creating some huge efficiencies over paper based systems.
The Health and Safety department has to collect this sort of data and collate lots of individual reports into senior management reports which this looks very similar to, so if you want to create a user friendly workflow for all contributing users it will save a great deal of time and mean your stats come in with little effort.
Or you could just keep it simple as suggested above and create a smartform to collect this info, but you will be missing some real benefits in not making it part of a workflow?

Creating a sheet where each point is its own column would (even though it would be a large sheet) give you the ability to create a form for users to input their data into. You can then use some simple formulas on a master sheet to collect the overall data counts based on date/quarter/year/etc. and/or use a report(s).
