I'm creating customer and employee satisfaction surveys that I want to send out as a web form. Does anyone know how to actually make them show up in the web form as stars??

It cannot be done.
I suggest that you either call the column Star Rating or in the form use the Alternative Name option to call it star Rating. In the description places some text to the effect " Select the number of stars you wich to award/Rate this service with2
