The team sets the dates themselves. Or i input them, it depends. I'm new to the project so I am trying to find the easiest way to simplify the process. Whoever speaks with the nurse will provide me with the info of the new dates.

Hi - can you share what you did? I have a request to create a sheet where a team member wants to input data, and then send a form link out for additional information to be collected. We would all of this information to be populated on one line, and it sounds like you possible got this to work if I'm understanding this thread correctly! Any help or advice you could provide would be so helpful! Thank you!

@Sheryl Sheldon It sounds like an Update Request may be a good fit for your specific situation. Your Team Member starts the row with their data then sends out an Update Request that arrives to the end users in a form like layout. When they submit the Update Request, all of the information is populated on the same row.
