Automation rule: Send request for udpate

Hello everyone

I would like to have automatic rule for sending request for udpate. Currenlty it works but it informs about delay only (se as an alarm).

I would like to have update form instead information in that mail. The problem is that I would like to have update form only with fields Task name, forecasted date and commments, nothing more. How to do this?

If you click the hamburger menu in the top right as shown below you can select what to include.
