Signatures on Forms
We would like to get employees to sign after they have been asked some questions via a Form. The Form will be filled in on an iPad or phone by Site Foreman and then the employee would need to sign acceptance. Is there a way to do this?

There are a couple of options.
- Save/export the form as pdf and sign on the mobile device
- Send out an Approval Workflow (if ok instead of the signature)
- 3rd party integration/service

Thanks for the tips. Unfortunately, I don't think either of these will work as I want the employees to sign on the site foreman's device on the form. They will be standing in lines waiting to be questioned, so it needs to be quick.

It sounds like a 3rd party option would be the best solution.
Would it work if all the forms were created on a pc and that they got signed on the mobile device?

Thanks for your prompt response. Below is the link to my basic form. It's a COVID-19 screening questionnaire that we will need to fill out for every person entering site just before they enter. I'm trying to streamline this process and also save some paper as there will be about 50 people per site per day. The site foreman would need to fill it out on his phone/tablet while asking the employee the questions, then we would like the employee just to sign at the bottom within the form, so that it is recorded that we have actually asked the employee and not just made up the answers. It however doesn't seem possible to do it in a simple way. A workaround could be to use a PDF application or similar instead.

A tip. If you use the Smartsheet Forms, you could have the Employee information pre-filled with either specific URLs in a list or similar or maybe by each employee having a QR code you scan to open their form.
