When you say it is not working... Are you getting an unexpected number or are you getting an error?

I see the problem... Your column name in your formula
[Third Choice (9-12 Months)]
is missing the colon in the column name.
[Third Choice (9-12 Months):]
That is the reason fort he error. Correct that and see if it is working for you.

Now, my question is when a person is going to make the selections on the form- how this number will change? Where i will see those counts from the voting? In the same space? I already built out the form and now my only concern is how this will work?

The counts will automatically update as new forms are added.

Currently it shows a count of 3, so when a new form is added the count will change from 3 to 4 or reset back to let's say 1 when a selection is made? And, also, by default the count is going to be 3 so what if someone didnt vote for that particular slection it will still show 3-how can we put a tab and know this was selected and this wasnt.

Subtract 3 from the COUNTIFS formula.
=COUNTIFS(..............................) - 3

Hi @Paul Newcome So far the form is doing great. One tiny thing that when i submit the form, it throws the data selected by the user at the bottom of the sheet. is there a way i can add those results or selections from the form to go on the top rows? Like, have the new submissions added to the first row of the sheet instead of the bottom row?

There is a section in the Form Settings that allows you to have the form populate either the bottom or the top of the sheet.
