Is anyone using an add-on to create forms. I need to have a form that will pull data from a sheet into the form before it is submitted.

I have customer data in a sheet. In a perfect world I would like to have a drop down in the form that is fed by the customer data. When the customer name is selected it would fill the form with the data from the customer sheet. I know this is not possible with out of the box features. As a work around I have published a Google sheet and edited the iframe to create a widget that will look up the data. Them I have created a SS dashboard and added the form and the Google sheet widget to the dashboard. The form user has to key the info from the Google widget to the form. Pretty lame workaround. I was looking for a form app or something that I could collect the data and then feed it to SS.
