MAX(COLLECT using INDEX(MATCH

Hi,
For the attached sheet excerpt I would like to get the latest Date of Visit that has a checkmark for a Re-Exam and matched to the Barcode @row. I have tried multiple MAX(COLLECT AND INDEX(MATCH variations with no success. Any help will be much appreciated.
