How do I create recurring questions to repeat a series of questions if the person selects yes?

How do I ask a question if the answer is yes then a series of questions will be repeated. For instance, If I ask, Do you want to add an additional widget?, then a series of questions will populate. Questions could be:

Color, Size, Weight, Dimensions, Number, etc.

Then we would ask the same question again, Do you want to add an additional widget? If the response is yes, the questions repeat. If the response is no, the form goes on to the next question or ends.

I used a form with conditional logic and when the question is asked the first time and the answer is yes the follow-up questions drop down to prompt the person completing the form. When the question is asked the second time and the answer is yes, the questions are not dropping down even though I have the same logic linked to it as before.

You'd need to add multiple columns for each grouping.
