Form Fields Out of Order

I have created a form with with about 30 fields. I have set up some automated update requests. When the person receiving the update request link opens up the link to the form, all the fields are not in the order I set them up in. Any ideas what has happened here?

In an Update Request, they will show in the same order as the columns in the sheet.
