Collating Data

Hi :)

I have a long form that asks a heap of questions and if the answer is "no" then more details need to be provided, i need to pull all of the extra details back into a different register but have them stacked vertically and separately (see example below) is there a way to do this ?

Snip 1 : This is what the form looks like

Snip 2 : How the extra details are collected

Snip 3 : How i would like it to show in different sheet (automatically populated)

Any help is greatly appreciated

How many groupings do you have in each form? Is it only those two?
