Requesting additional actions after approval is granted. (Need to select a charge account)

Options

Employees will select a shop they work for when requesting on the form. The shop determines which manager needs to alerted to approve/decline the request. If the request is approved, I need the manager to ALSO submit a charge account for the request to be charged to. Trying to get this done in one email, which is to approve/decline and submit a charge account from a hidden field in the form.
