IF/AND/VLookup Formula Error

I am trying to create a formula that requires a vlookup to another sheet. The formula reads:

=IF(VLOOKUP(Address@row, {Ranking Matrix Range 3}, 16, 1) < 28, 100, IF(VLOOKUP(Address@row, {Ranking Matrix Range 3}, 16, 1) = 28, 50, IF(AND(VLOOKUP(Address@row, {Ranking Matrix Range 3}, 16, 1) >= 29, <39), 40, 0)))

Essentially, I need the formula to determine if a number the vlookup is returning is the following:

Less than 28 = 100

28 = 50

Between 29-39 = 40

Can anyone help? This is returning a Invalid Operation message and I know it is because of the IF(AND(VLOOKUP
