1st question in Smartsheet Form isn't 1st column in the answer form

I have a Smartsheet Form that asks for the person's name and region first, and then there are additional questions, some with logic behind them. The beginning questions are all required, the ones with logic are not. The first 2 questions are showing up as much later columns in my answer sheet, and I don't understand why?

I realized that the Your Name field was not the primary for some reason, and the sheet is live so I'll leave it as is.
