This formula works until I add the IFERROR and the parenthesis before the word INDEX and after the last )
=IFERROR(INDEX(COLLECT({EEO/HARRASSEMENT}, {NAME}, NAME@row, {EEO/HARRASSEMENT}, <>""), 1))

You need to tell it what to output if there is an error.
=IFERROR(INDEX(COLLECT({EEO/HARRASSEMENT}, {NAME}, NAME@row, {EEO/HARRASSEMENT}, <>""), 1), "what gets output in the event of an error")
