show automated information on form

I have created a form to capure any issues on a particular site.

The issue number is auto generated, but I can't get the number to appear on the comfirmation email /form.

Any help would be appreciated

You must add a Workflow with an Alert to the sheet and include the Issue Number.

Make sense?

Andree you're brill, but I think thats a little more complicated than I need.

I just need the autogenerated number to appear on the form. I have put the field on the form, but its not showing

As you can see I can get the column header to appear, but not the info.
