How do I fix this formula

I need to this column to populate with the symbol from column Health if Customer Dashboard column has a checkmark. What am I doing wrong here?

@Andrée Starå looks like I had to change Yes for Customer Dashboard checkbox to 1. That made the formula work.

@Andrée Starå I had to fix the symbol in the customer dashboard to match the Health Symbol and I still could not get a value to return.
