Forms: Logic to force conditional selection to a new row.

Hello, in Forms I am trying to have a conditional Yes / No selection place the yes information on a separate row while carrying over or duplicating fixed elements of the form.

EX.

Name: Chris

DO you like Ice Cream - Yes or NO

If Yes -

What Flavors? Choc Van Straw

Staw

Are there any other topping you like? Yes or NO

If Yes -

Please explain what you like

With butter finger

Thank you for submitting form

If No -

Thank you for submitting form

In this scenario - I would like the favors to hit a new row like normal and the toppings to create a new row carrying over the name.

    This might help!

    I developed a solution using two forms (or the same form by using conditional logic) that can be used to submit the main information and then the other information on a new row for each submission after the first.

    Would that work/help?
