Vlookup In Form

I have read through some answers and can't find what I am looking for. I want to create a form that when a certain name is selected, it prepopulates the remaining fields BUT allows the option to override meaning it can't be a true vlookup as it locks the field. AND I need this option avaliable within the form prior to submission. Is this possible?

Ie. I select my name in the form, it populates my number, address, and email address but allows me the option to override it to someone elses info if necessary.

This is not possible in Smartsheet forms. You would need to look into incorporating a third party form service that would allow that.
