Alert not sent to contact in a cell

Hello all,

I'm trying to automate a process where an alert is sent to a contact in a cell.

When testing it won't seem to work. Do I miss something?

Input for "E-mail" is a form.

I hope someone can help me out.

Have you checked your "Personal Settings" to ensure the box is checked that allows notifications to be sent to you regarding your own changes?
