Hi, is it possible to have multiple fields side by side on one line? Or must each field have its own line.

Unfortunately, it's not possible now to have fields side by side, but it's an excellent idea!
Please submit an Enhancement Request when you have a moment.
As a possible workaround, you could use a 3rd party form integration.
Is that an option?

Yes, of course.
Here's a couple to test. If they don't have a direct integration, you can use Zapier to connect them to Smartsheet.
- JotForm
- Formstack
- Wufoo
Would any of those options work?
