Is it ok to assume that this is a master sheet and you are looking for the count in another sheet where you have the Associate Name column and Column for Month that will be used to check the criteria?
If not then I would recommend creating helper formula column for month based on "Entry Date", and then create a report where you can group by the new month column and associate name, and summarize by associate name.

Adding to Sameer's advice above, the formula below could be used if you would like to see the information in the sheet itself.
