Repeating form questions

I am creating a form and need to ability to ask the same questions multiple times (i.e. repeat a column name). Is there any way to do this without having to add 1, 2, 3, etc... at the end of each Field Name?

Columns must have unique names on your sheet (i.e., Question1, Question2, etc.); however, you can change the label on forms to whatever you want it to be. Labels can be repeated. Hope this helps.
