Locating Form Submissions
The form is active as colleagues have told us they have submitted, but we have no idea where to find the results. The form owner is no longer available. How do we locate the results?

The form should link back to a sheet within Smartsheet. The admin for your Smartsheet licenses should able to locate the sheet that the form belongs to so you can share it with someone else that is available. I would also recommend, Always have a back up!

Our system admin is new. How would this person locate the sheet?

You will need to reach out to Support. There is no way for us end users to map a form URL back to a specific sheet.

Just jumping in here to confirm that @Paul Newcome is correct 🙂If you only have the published form URL, and the form creator did not add a link or information about the underlying sheet, then the only way to find that related sheet is for someone who is shared to it to find it in their Smartsheet instance.
You can use Search to filter by Form Names in case your account has access:
Otherwise, if no one has access to that sheet and Form, then yes, you will need to contact Smartsheet Support with the form URL. If that form is associated with a sheet in your plan, then they can tell you the sheet name and possibly the owner.
