Hi, I'm creating a Request form for teams to be able to order either a individual item or multiple items and quantities. I'm trying to figure out the right approach. i.e. Select item type / Select the quantity. I want the form to be simple and not have to 10 different lines for each item ... any recommendations?

Is it possible to use one form from Sheet1 to create line item in Sheet1 , Sheet2 , Sheet3 etc
