It would be helpful to have the option for multiple file upload fields in forms, for example for photos, so that they could be associated with particular responses and not grouped together.
I need to be able to have people upload multiple documents in a form submission. In my case I need to ask for a copy of their driver's license and I also need a copy of their insurance card. I think it's very inconvenient to not be able to have multiple upload fields.
To have the person need to combine their two (or more) documents into one in order to fill out the form seems very cumbersome especially if they aren't as tech savvy. Please make this possible.

there would, however, still be some utility in having multiple attachment fields. even though all the attachments would be dumped into one row in the end, there have been times stakeholders wanted to present a form with separate required fields for "upload x certificate here," and, "now upload scope of work document y here." so the end user doesn't miss uploading parts of the documentation being collected.
Hi - Can the feature/option for Multiple File Upload Fields in a form be added? This will allow correct documents to be added based on the logic created & order of form.
I like using Smartsheet for public forms because they have a much cleaner look than other options. However, other options allow for different file uploads. My form asks for different documents, and it is best for the user experience when they have a specific place to upload each document so documents are not missed. Currently, I can only use one upload file in an entire form. Knowing how the responses look in a sheet, this makes sense. But there must be a way that we can have multiple file upload boxes in a form, even if they all load into the same place in the sheet.
I agree - super useful feature that is needed. I can't move one of my surveys off JotForm because it provides that capability, while SmartSheet does not
Please add the option of having more upload fields on one form.

Would love this feature also. Our current workflow requires specific documents (and more than 10, which has been the limit since 2016!!!!).
It would be great if upload fields would allow us to use the upload field label as the default file description after upload. This would be an easy way to classify incoming documents and trace them to which form field was used.

Multiple upload fields would definitely make forms easier to organize, especially when people need to upload different files for different questions.
For photos, though, there’s another piece to think about. If the photo needs to show up in a report, dashboard, or generated PDF, it usually needs to be in an image cell — not just sitting with the row attachments.
That part can be handled today with the API, or with a workflow that moves form photos into the right image cells.
A simple version would be:
Form photo → image cell → report/dashboard/PDF
So for general document uploads, this roadmap item makes a lot of sense. For photos that need to actually show up inside Smartsheet outputs, the image-to-cell step is the part I’d look at.
